Health, Housing & Social Services Referral Scheme

Are you a healthcare professional?
If you work in the health, housing or social services fields, and you visit clients in their homes to provide services, you can make use of our referral scheme. It provides you with a series of simple forms to make a note of any problems or issues you find when visiting clients - e.g. if their home needs repair, there's a noise or anti-social behaviour problem, or they have health problems.
How to use the referral scheme
- In all cases the referral form should be completed and attached to the client's file. The referral form is top of the list of 'printable forms' below;
- Where the problem has been classed as severe you should refer the client on immediately by telephone to the correct agency - all the contact details can be found in Checklist & Referral Directory (second item under 'printable forms');
- If the problem is moderate then, depending on the client, you can either make the referral by calling the right agency (see the Checklist & referral Directory, second in the list under 'printable forms'), or by referral card - this is third in the list under 'printable forms' below).
- If it is a non-urgent case you should fill out the referral card and leave it with the client.

About the scheme
The directory was developed following consultation with Health and Housing Services. Professionals found that during their visits they were coming across problems outside of their field, and were recognising their lack of knowledge of services and contact details available.
The directory therefore aims to make people aware of the range of services which are available. A checklist of problems is included, along with a list of relevant services and contact details.
It is hoped that the use of the directory will encourage better understanding of the range of help which is available and also to ensure that people are being linked into the range of services they need, through improved communications between services.
The directory therefore also provides paperwork to help professionals and their clients make referrals to the services included in the directory.
